Ordering Information

Acknowledgement of Orders: Acknowledgements of all accepted orders will be mailed to our customers. Responsibility to check factory's interpretation for correctness is the customer's responsibility. Complete and clear specifications, along with a signed purchase order, must be received for orders acceptance.

Prices: All quotations are distributor net cost, and are subject to change after 30 days. The right to refuse orders is reserved, and access to this web site does not guarantee order acceptance.

Credit Policy: Orders placed by new accounts are subject to prior credit approval, or cash in advance. If an account has a balance more than 15 days past due, open account privileges will be revoked and orders in process will be placed on hold.

Terms & Conditions: 3% discount on invoices date 1st thru 15th, paid by the 25th. 3% discount on invoices dated 16th thru 31st, paid by the 10th. 30 days net past due thereafter, LATE CHARGE OF 1 1/2% PER MONTH.

Overruns & Under Runs: All orders are subject to overruns and under runs of 10%. If exact quantity or smaller limitations are requested, an up-charge will be applied.

Cancellations: Accrued costs, plus $20.00 will be charged on cancelled orders.

Complaints & Returned Goods: All complaints should be made within reasonable time of receipt of goods. The factory will not accept any returned goods without prior approval. The limit of responsibility is the replacement or credit for the goods & freight costs.

Responsibility: Calibrated Forms Co. will not assume any responsibility for the legal implications of the substance, wording, or format of any form.

F.O.B.: All orders are shipped F.O.B. the factory. All orders are shipped prepaid unless otherwise specified. All orders picked up or delivered to the carrier constitute delivery and transfer of title and possession to the buyer. The factory is not responsible for goods damaged in shipment.

Reorders: Reorder copy held by the factory will be maintained for 36 months from the original date of shipment. All negatives furnished will be kept for 36 months unless requested returned.

Form Margins: Equipment specifications require a minimum of 3/8" lockup on continuous forms depth, and unit set width.

Factory Job Numbers: All correspondence and telephone conversations should be referenced by the factory job number.

Copy: Acceptance of camera-ready copy, or negatives, is subject to strict factory approval.

Composition: Composition Guidelines listed below.

Packing: Packing specifications will be factory option. Our standard is bulk pack in 200lb. test 11" high cartons. Special order cartons will be available as an up-charge only.

Deliveries: Calibrated Forms Co. will have two (2) delivery classifications designed to meet your service needs and still maintain the scheduling flexibility needed to keep our prices at a very competitive level.

Rush Delivery

The distributor can expect the orders to ship on the date acknowledged, unless notified by Calibrated Forms Co.

Normal Delivery

The distributor can expect the orders to ship within two (2) working days of the date acknowledged, unless notified by Calibrated Forms Co.

General Delivery Guidelines

Note: All rush deliveries need to be pre-approved. Orders held for questions or other reasons over one day will have the delivery date adjusted outward the number of days on hold. Add five (5) work days for Unit Sets that book.

Composition Guidelines

Reorder of Negative Furnished - the negative is complete (including screens). Digital copy included.

Negative Furnished / with Screens - the negative is complete except screens will have to be added.

Camera Ready Artwork - copy must be produced in a professional format and will require no changes.

Camera Ready Artwork / with Screens - same as previous line except screens will be added.

Shoot Sample - any previously printed form with no screens.

Shoot Sample / with Screens - same as previous line excepts screens will need to be added.

Special Notes

The above criteria are based on no alterations.

Alterations can be quoted on any of the above criteria. Describe alterations as:

A. Limited: These lines of type or less need change with no changes in ruling.

B. Minor: 5% or less of copy is affected.

C. Medium: 5% to 20% of copy is affected.

D. Major: Over 20% of copy is affected.

When orders are quoted "shoot sample", Calibrated will reset the form at quoted price if quality is not satisfactory after camera.

Proofs will be provided upon request and on jobs we deem necessary.
